Supplementary Information Form (SIF)
Despite the instruction on the Herts application form, Supplementary Information Forms are only required for pupils applying under the Musical Aptitude Test criterion or Children of Staff criterion. A SIF is not required for all other applicants not applying under the aforementioned criterion.
If you are applying under the above criterion, please download and print the below SIF and return it c/o Admissions Officer, Croxley Danes School, Baldwins Lane, Croxley Green, WD3 3LR. This can be emailed to admissions@croxleydanes.herts.sch.uk, sent via post or hand delivered to the postbox situated on the wall outside of school on the left-hand side of the main entrance.
